What to Expect

    1. Select Your Appointment Type: Choose the appointment type that best fits your needs. Clicking on each appointment will provide additional details for that appointment to help guide your decision. If you're uncertain which type to select, please email me at pt@hoopersbeta.com.

    2. Select an Enhanced Rehab Tier (Optional): While booking, you'll have the chance to add Tier 1 or Tier 2 of the Enhanced Rehab option at an additional cost. More information about the two tiers available on the main 1-On-1 Physical Therapy webpage.

    3. Complete the Intake Form and Waiver: After selecting the appointment type, please be sure to fill out the required intake form and waiver form.

      For discounts, please input the code before completing payment. For more information on discounts offered, see "additional notes" below.

    4. Get Ready to Meet: I’ll review the information you submitted and contact you if any further details are needed.

      • For online appointments: We’ll meet through Google Meet. The link for this will be sent with your booking email.

      • For in-person appointments: I’ll see you at The Wall!

    1. Review the Intake Form: We’ll begin by reviewing your intake form and gathering additional information about your injury.

    2. Begin the Comprehensive Evaluation: We’ll assess all contributing factors, including both physical and mental components. As we identify issues, I will provide detailed explanations to ensure you fully understand everything discussed.

    3. Finish with a Review and Discussion: We will have a dedicated time to answer any questions you may have to ensure all your concerns are addressed.

    1. Receive a Detailed Report: I will prepare an assessment and treatment plan, which will be sent to you for your records.

    2. Receive a Home Exercise Program (HEP): You will receive a treatment plan that includes access to a Home Exercise Program with videos and explanations of exercises or stretches. Note: If you are signing up for an Enhanced Rehab Plan, your HEP will be shared with you via the app!

    3. Discuss Follow-up Appointments: We’ll discuss whether or not follow-up appointments are needed to monitor progress and make any program adjustments.

Additional Notes

  • I am considered an "out-of-network" provider, which means I don't have a direct agreement with any insurance companies. Because of this, I do not bill your insurance directly. Instead, you pay for my services out-of-pocket.

    However, if your insurance plan includes out-of-network coverage, you may be able to get reimbursed for some of the cost. After you’ve paid, you can request an invoice, which you can then submit to your insurance company for potential reimbursement. The amount reimbursed (if any) will depend on the specific details of your insurance plan.

    Please note: Invoices are provided upon request only, and reimbursement is available only for services provided to clients in California—whether in-person or online. If you live outside of California and use my online services, insurance companies will not reimburse you.

    💻 Online: Allows more flexibility in scheduling. I have extensive experience helping clients through online appointments, ensuring effective, personalized care remotely.

    Initial Appointments (In-Person or Online):

    • Comprehensive evaluation and discussion of your injury, training, and goals

    • Thorough review of findings and anatomy education

    • Complete personalized treatment plan

    Follow-Ups (In-Person or Online):

    • Progress review, treatment plan adjustments, and time to address new or unresolved concerns
